I, the undersigned, acknowledge that participation in The Art of Performance organized by Transcendence Productions Inc. involves inherent risks, including but not limited to the risk of physical injury, death, and loss or theft of personal property. I understand that these risks cannot be eliminated without jeopardizing the essential qualities of the activity.
In consideration of being permitted to participate in The Art of Performance, I hereby agree to the following:
1. Assumption of Risk: I voluntarily and knowingly assume all risks associated with participation in The Art of Performance, including but not limited to those mentioned above.
2. Release and Waiver: I, on behalf of myself, my heirs, executors, administrators, and assigns, hereby release, discharge, and hold harmless Transcendence Productions Inc., its officers, directors, employees, agents, and volunteers from any and all claims, demands, actions, or causes of action of any kind arising out of or in connection with my participation in The Art of Performance, including any claims for bodily injury, death, or loss or damage to personal property.
3. Indemnification: I agree to indemnify and hold harmless Transcendence Productions Inc. from any and all liabilities, losses, damages, costs, or expenses (including attorney's fees) arising from any claims made by myself or any third party as a result of my participation in The Art of Performance.
4. Medical Treatment: I authorize Transcendence Productions Inc. to obtain medical treatment for me in the event of an injury, accident, or illness during my participation in The Art of Performance. I understand anda gree that I am responsible for all medical expenses incurred.
5. Photography/Video Release: I grant Transcendence Productions Inc. permission to use my likeness in
photographs and/or videos taken during The Art of Performance for promotional and marketing purposes
without compensation.
